Please make sure your case meets the following criteria

You are the missing child's legal guardian * The child is between the ages of 11 and 16

The child has been missing for 90 days or less 

There is an open missing person case with a law enforcement agency

Things You Will Need To Fill Out The Form

Police agency name, number and case number.

Child's date of birth, height and weight.

Please be prepared to submit one or more recent photographs of your child.
